<trans-abstract xml:lang="en"><p>Crohn's disease is a chronic, relapsing gastrointestinal disorder of unknown etiology, characterized by transmural, segmental, granulomatous inflammation with the development of local and systemic complications. The disease progresses with alternating periods of remission and exacerbations and may be accompanied by the development of strictures, fistulas, abscesses, intestinal bleeding, extraintestinal manifestations, decreased quality of life, and the risk of disability.</p><p>These guidelines address the management of follow-up care for patients with Crohn's disease in primary care. The document presents the basic principles of patient registration for follow-up monitoring, the frequency of follow-up visits, and an algorithm for assessing complaints, medical history, physical and paraclinical examination methods. Particular attention is paid to monitoring disease activity, treatment efficacy and safety, assessing the achievement of health targets, promptly identifying unfavorable prognostic factors and indications for hospitalization, as well as preventive measures. Follow-up care is considered a key element of proactive patient management aimed at maintaining steroid-free remission, preventing complications, reducing the hospitalization rate, and improving long-term clinical outcomes.</p><p>These guidelines are intended for internists, primary care physicians, general practitioners, and paramedics performing the doctor’s functions. They may also be useful for gastroenterologists, proctologists, healthcare executives, and other specialists involved in the management and monitoring of patients with Crohn's disease.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>болезнь Крона</kwd><kwd>диспансерное наблюдение</kwd><kwd>первичная медико-санитарная помощь</kwd><kwd>воспалительные заболевания кишечника</kwd><kwd>методические рекомендации</kwd><kwd>врач-терапевт</kwd></kwd-group><kwd-group xml:lang="en"><kwd>Crohn's disease</kwd><kwd>follow-up monitoring</kwd><kwd>primary care</kwd><kwd>inflammatory bowel disease</kwd><kwd>guidelines</kwd><kwd>general practitioner</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Хавкин А. В. и др.
